Capitol Hill Care Link helps older adults (age 60+) residing in Denver's Capitol Hill neighborhood maintain their independence, health, and quality of life by providing social connection, resources, and service referrals that fit their unique needs.

How it Works
Capitol Hill Care Link works with you to determine what your needs are. Whether you need assistance enrolling in or changing your Medicare plan or you just need a little work done around the house, we can help you find the right person for the job by providing referrals to service providers who can meet those needs.
We also provide community members with opportunities to participate in social, educational, and volunteer activities. We work closely with The Center and our other community partners to offer activities like yoga, SilverSneakers® yoga, storytelling, coffee & conversion, art, and outings throughout the city.

Our mission is to build community and connect adults 60+ in Capitol Hill to services, programs, and resources needed to thrive at home safely and independently for as long as possible. We offer a single referral source for comprehensive services for adults:
Health & Wellness
In-home health screenings, counseling, wellness coaching, exercise, nutritional assistance and education
Help navigating systems like SNAP, Medicare, Medicaid, HCBS, LEAP, Old-age Pension, and more
Affordable homemaker services, home health care, home repair & modification, and in-home safety evaluations
We partner with several different organizations to help you find transportation to get you where you need to go

25% of The Center's visitors participate in one of our programs for older adults—come join our warm, supportive community. Stop by The Center to attend one of the many social, educational and support programs that we offer through the SAGE of the Rockies and Capitol Hill Care Link programs.
